Spiritual Care Jobs in Macon, TN at Addus HomeCare can include:
Hospice Chaplain – The Hospice Chaplain takes on a variety of functions including
- Performing spiritual bereavement assessments of patients and families/caregivers in the hospice program in a timely manner and ensuring that identified needs are addressed in the patient’s plan of care
- Providing direct spiritual care and bereavement counseling to patients and families/caregivers
- Demonstrating exceptional service to patients, families, referral sources, co-workers, and other customers. Assisting team members as needed to ensure that this level of service is consistently provided
- Providing accurate and timely documentation of patient services to reflect the plan of care.
- Attending group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
- Providing consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling
- Participating in Continuous Care and on call, if necessary
- Assisting in supervision of spiritual care volunteers when assigned to patient’s families/caregivers as needed
- Assuming responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; making available bereavement services for up to one year following the death of the patient
