The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. What You’ll Do:

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
• Must be knowledgeable of quality assessment and assurance procedures
