Seeking:
- LPN for in-home care for private duty client in St. Clair Shore, MI
- Part-time between 12-15 hours/ week

Some of our requirements:
- Licensed Practical Nurse (LPN) through the state of Michigan with current and unrestricted license.
- 6 months recent, relevant experience is preferred (in Home Health Care)
- Assess/ evaluate s patient needs, reports changes and patient status to physician
- Updates Care Plan
- Submits documentation thoroughly and timely
- Supervises Home Care Aide (as applicable)
- Experience with skin management, medication management, general health assessments, care coordination.
- Strong wound care skills
- Must be able to pass a background check
- CPR certified preferred
- Reports to Registered Nurse and Agency Director
