Essential Duties:
The Clinical Liaison is responsible for collaboration with the Community Liaison team and Admissions team to increase public awareness of hospice and other services provided by the agency and to drive admissions for the placement of patients to meet expectations. This role will act as first point of contact for hospitals, health care facilities, and other sources regarding referrals, will seek networking opportunities to provide education and to develop relationships with new and existing referral sources. In addition, this role will be responsible for identifying appropriate care and services to patient and family in accordance with attending physician and plan of care, as well as company policy and procedures.
